1. Treatment and purpose TCHP: docetaxel, carboplatin, trastuzumab and pertuzumab · NCCP 00722 v2
All four medicines are given by infusion on Day 1 of a 21-day cycle, for six cycles before surgery. Surgery findings guide the postoperative treatment. There is no routine take-home chemotherapy pump. After surgery, the pathology helps select the next HER2 medicine. The plan usually aims to complete about one year of HER2 treatment from the first dose. Residual invasive disease may change the treatment. Blood tests and supportive medicines must follow the selected postoperative regimen.
Review the treatment journey, tests and phase transitions with the patient.
Individual aim, expected benefit, uncertainty and assessment plan
Reasonable alternatives, including supportive care and not proceeding with this regimen
2. Risks and supportive care Docetaxel · Taxane chemotherapy Take prescribed steroid premedication. Report fever, lasting numbness, swelling, sore nails or infusion symptoms.
Carboplatin · Platinum chemotherapy Keep blood-test appointments. Fever, unusual bleeding, repeated vomiting or infusion symptoms need prompt advice.
Trastuzumab · HER2-targeted antibody Attend planned echoes. Report breathlessness, ankle swelling, rapid weight gain or infusion symptoms.
Pertuzumab · HER2-targeted antibody Report diarrhoea early, especially with poor fluid intake. Keep cardiac checks and report infusion symptoms immediately.
These are selected discussion prompts, not a complete risk list. Discuss serious, potentially life-threatening and lasting effects; tailor the conversation to comorbidities and the patient’s priorities. Discuss the treatment aim, alternatives, expected benefit, common and serious effects, fertility and pregnancy precautions, route, monitoring, practical support and the agreed contact plan. The treating team completes the approved individual consent process.
